Shared decision making puts the patient at the centre of decisions about their care and treatment. It is a collaborative process where a clinician, such as a GP or hospital consultant, supports a patient to reach a decision about their treatment.

NHS Birmingham and Solihull has launched a suite of communication resources to promote shared decision making to our citizens.

The resources aim to raise awareness of shared decision making by explaining how decisions about a patient's healthcare, such as choosing between treatment options, should be a collaborative process between the healthcare professional and the patient, and that shared decision making can help a patient to reach the right decision for them about their physical and mental health. All of the resources are available in the Campaigns and Toolkits section.

Personalised care and support planning encourages healthcare professionals and patients with long-term conditions and their carers to work together to clarify and understand what is important to the patient. They agree goals, identify support needs, develop and implement action plans, and monitor progress. This is a planned and continuous process, not a one-off event.

In March 2023, NHS England launched a Personalised Care and Support Planning Framework to support integrated care systems (ICSs) to understand and create the conditions for sustainable implementation of personalised care and support planning.

In line with this, our Personalised Care Team have developed a draft personalised care and support plan (PCSP), which can be used by teams looking to implement personalised care and support planning. The draft PCSP can be used in part or in its entirety.

A personal health budget is an amount of money allocated to an individual to help them meet agreed health care outcomes.

In March 2023, NHS England launched a Personal Health Budget Quality Framework to support the implementation of personal health budgets (PHBs) across integrated care systems (ICSs) to build on best practice and realise the improved outcomes and experience of NHS care and support that PHBs can offer for people.

Birmingham and Solihull ICB policy for Personal Health Budgets can be found on the NHS Birmingham and Solihull website.
